About Your Case Manager
Your OSU HAN Case Manager is a registered nurse who will work with you and your family or caregiver to look over your health care needs and help locate the resources needed to keep you healthy. We help you identify what is important to you and the best way to reach your health care goals.
What Case Managers Can Do For You
- Provide extra support for SoonerCare Choice members who have been identified by the Oklahoma Health Care Authority
- Provide special need for case management services
- Assist with managing and coordinating health care services
- Receive referrals from your primary care provider
- Provide education to help you gain the confidence, knowledge and tools
- Works with your primary care physician and specialists to set goals and treatment plans
- Teach you about your diagnosis or current illness
- Provides tools and resources to help you live a healthier life
