Application Information

This program is not participating in the APPCN Match and all applications should be emailed to the Program Coordinator, Camillia Estes at k.estes@okstate.edu

 

Applicants are required to meet one of the following qualifications to be eligible to interview:

 

For the 2018-2019 academic year, we will be recruiting 2 fellows. In order to be eligible, applicants must have:

  • Completed a CPA- or APA-accredited Clinical Psychology, School Psychology, Counseling Psychology or Clinical Neuropsychology doctoral program
  • Completed a CPA- or APA-accredited pre-doctoral internship with major rotation in Clinical Neuropsychology
  • Have substantial experience in clinical neuropsychological assessment of children or adults

All degree requirements must be completed before the start date.

Electronic applications are preferred. Applications must contain the following 8 items to be considered complete. With the exception of the official graduate school transcripts (#6), director of training statement (#7), and reference letters (#8) being sent directly to Dr. Mazur-Mosiewicz, applicants should put items 1-5 in a SINGLE email or envelope in this same order.

 

If sending application via email, please use the subject, “Post-doctoral fellowship application package”. Conversion of documents to PDF is encouraged to maintain desired formatting when sending via email. In the package, please include the following:

  • Letter of interest
  • Curriculum vitae
  • Two sample de-identified neuropsychological reports
  • Copies of representative publications (if applicable)
  • Application form and checklist. Application can be downloaded from our website
  • Official graduate school transcripts (sent directly from school to Dr. Mazur-Mosiewicz)
  • A statement from your graduate school clinical training director indicating your status in the program and probability of completing all doctoral requirements prior to the start date of the fellowship (the latter is waived for those already holding the doctorate) (sent directly from training director to Dr. Mazur-Mosiewicz)
  • Three letters of recommendation (sent directly from referees to Dr. Mazur-Mosiewicz)
  • Email the information directly to anna.Mazur@okstate.edu

Interviews

After initial review of the application packages by the admissions committee, potential candidates will be contacted via email to arrange an interview. Interviews will be conducted at 2018 INS Meeting in Washington, District of Columbia, USA (February 14-17). A candidate is welcome to travel to OSU-CHS for an in-person interview, but this is not necessary to be considered for the position. Interviews using Skype or telephone can be considered for applicants unable to travel (Skype is preferred).

 

Audition Rotation

Please visit the Office of Clinical Education to review the following:

  • Complete the visiting student application
  • Upload the required documentation
  • Proof of Completed Compliance Training (eg, HIPAA, ACLS/BLS, TB test, Mask fitting)
  • Letter of good standing
  • Current CV and photo
  • Immunization verification form
  • Background check report
  • Certificate of malpractice liability coverage
  • Proof of health insurance coverage
  • 10 panel drug screen report
  • HIPAA/BBP training certificates
  • Letter of intent for Family Medicine
  • Passing your COMLEX
  • Verify that an affiliation agreement is in place with your school and OSU

Once these items have been uploaded, your file will be forwarded to the Department of Clinical Neuropsychology for review.  An email will be sent to the student with an approval or denial of their request.

ERAS Information

Not participating in match at this time.

 

Non-DO Applicants

20 Business Days Prior to the Match the Applicant Must Have Ready for Submission:

  1. 60 hours of OSU-COM’s years one and two to include; OMM I, II, III, and IV. A Teaching Guide for Osteopathic Manipulative Medicine or lectures may be used to direct the lab portion. Certificate of completion will be provided.  OMM education from outside institutions will not be accepted.
  2. Verification that candidate completed the CORRE examination with a minimum passing score at or above the 10th percentile.
  3. Letter of recommendation from a D.O. with an active practice to be included with residency application. (letter must indicate that applicant understands osteopathic philosophy)

During Residency:

  1. Complete 60 hours of OPP Continuing Medical Education, approved by the OSU-CHS/OMECO, achieved through any of the following: CME courses, didactics, elective rotations, etc. focusing on clinical application.
  2. Successfully pass an OMM basic skills competency practical examination, administered by OSU-CHS faculty, prior to PGY-2.
  3. Satisfactorily progress through the Osteopathic Recognition Milestones during residency.
