Congestive Heart Failure

ECHO uses videoconferencing to connect an interdisciplinary team of experts with primary care providers. They collaborate in interactive case-based learning sessions to help providers develop advanced skills and best practices.

 

Who Should Participate?

Medical providers (DO, MD, NP, PA), nurses, clinical pharmacists, nutritionists, dietitians, psychologists, behavioral health providers, social workers, case managers, and other professionals interested in learning new skills.

 

Schedule

This ECHO Program is held the 2nd and 4th Mondays of the month from 12:00pm – 1:00pm CST. A one-time registration is required to participate in any ECHO sessions.

Why Congestive Heart Failure?

  • Approximately 6.7 million Americans over 20 years of age have heart failure (HF), and the prevalence is expected to rise to 8.5 million Americans by 2030. The lifetime risk of HF has increased to 24%; approximately 1 in 4 persons will develop HF in their lifetime.
  • From 2010 to 2020, HF mortality rates have increased for Black women and men at a rate higher than any other racial or ethnic groups, particularly for individuals below the age of 65.
  • Heart failure remains a leading cause of morbidity and mortality globally. Heart failure is a growing health and economic burden for the United States, in large part because of the aging population.
  • There is a large percentage of heart failure patients who are not being prescribed guideline-directed-medical-therapy. These therapies, if utilized, can result in improvements in survival and morbidity and mortality.
  • Individual patient care can be fragmented between multiple health care providers leading to inadequate screening, diagnosis, and treatment.
